How do I set up and use Vendor Audit Questionnaires?

Complete due diligence when onboarding new Apps and record the information directly in Setyl.

Vendor Audit Questionnaires help you complete due diligence when using new vendors, and record the information directly inside the Setyl platform.

You can create your own, customized questionnaire(s), and require users to fill these out when onboarding new Apps into the platform.

Step 1: Create and customize your Vendor Audit Questionnaire(s)


You must be an Owner to access the below features and set up the questionnaires.

  1. Log into Setyl.
  2. Go to Settings > Company Settings.
  3. Select the Vendor Audits tab along the top menu.
  4. Make sure the Enable or Disable App Onboarding Questionnaires toggle is switched to Enabled. This will make the questionnaires a requirement when onboarding an App. Note that, if you disable the questionnaires using this toggle, the questionnaires will be hidden from the App onboarding workflow, but your existing questionnaire content will be saved.
  5. You can now edit and/or delete the default questionnaires provided by Setyl, and add your own by clicking the Add New Questionnaire button, to fit your organization’s specific requirements. Note that deleting a questionnaire will permanently remove the answers from all Apps where it has been completed.
  6. When adding or editing a question within a questionnaire, you can choose whether to make these mandatory. You can also include help text to guide the end user.

Step 2: Onboard an App using your Vendor Audit Questionnaire(s)


Once the questionnaires are enabled, the below features are available to Owners, App Business and Technical Owners, and any Manager or Supervisor with the required permissions.

  1. Log into Setyl.
  2. Go to Apps > All Apps > Onboarding.
  3. Locate the App you wish to onboard and open its profile.
  4. On the first Onboarding tab, you’ll find your vendor audit questionnaire(s).
  5. Click Review to start filling out each questionnaire. Changes will be saved automatically as you type.
  6. Select Approve Answer to approve and lock in each answer, and prevent accidental changes.
  7. Once you’ve filled in your answers, click Complete Questionnaire. Note that all mandatory questions need to be filled in before you can complete your questionnaire.
  8. You can find and edit completed questionnaires under the Vendor Audit tab of your Apps’ profiles.
